1. The Florida Parole and Probation Commission shall use the following form in making applications for an extension of time within which to file presentence reports in accordance with Administrative Order No. 74-8. The form should be completed, signed by the appropriate supervisor and submitted to the judge who ordered the report. When the request has been acted upon, the original shall be filed with the appropriate clerk and the court secretary shall furnish a copy to the Commission and to the Court Executive Assistant.
